Medication Refill Policy

Medications will not be filled outside of your regular scheduled follow up appointment.

Patients are required to use ONE pharmacy.  If you change pharmacies, please report to office to record new pharmacy in your chart.  Medication will only be sent or called into the pharmacy of record in your chart.

Office Hours

Office is open Monday through Friday 7:30am to 4:30pm.

Office is closed during our lunch hour from noon to 1pm.

If phones are forwarded and you need to reach a physician please leave message and the physician on call will be forwarded your message.  Weekend’s the physician on call will return your call.

How to contact us

Paragon Pain & Rehabilitation will roll out a patient portal which patients will be able to access from our website.  www.paragon-ppr.com.  Patients will be able to complete a portion of paperwork required on initial visit, and can download the new patient paperwork by clicking on our website.

Emails sent to info@paragon-ppr.com will typically be answered within 24 hours.  Feel free to email request to schedule or change your follow up appointment at any location.  Also, feel free to email any questions you may have for your physician.  New patients can request appointment via email as well.
